Emergency SMS Service Description

By providing my mobile phone number and signing this form, I consent to receive automated SMS messages from Onamac Industries regarding emergency, safety, facility closure, weather, evacuation, power outage, and other urgent operational notifications affecting company operations or employee safety.

Message Frequency: Varies based on emergency conditions. Messages are sent only when necessary.

Message & Data Rates: Standard message and data rates may apply depending on your mobile carrier and plan.

Help & Opt-Out: Reply HELP for assistance. Reply STOP at any time to stop receiving emergency SMS messages.

Employee Consent

By checking the “I consent…” box below, I acknowledge that:
• I voluntarily consent to receive automated emergency SMS notifications from Onamac Industries.
• I understand message frequency varies based on emergency conditions.
• I understand standard message and data rates may apply.
• I understand I may opt out at any time by replying STOP.
• I understand I may request assistance by replying HELP.
• I understand this consent is not a condition of employment.

Employees may also revoke their consent by notifying Human Resources, who will remove them from future emergency SMS notifications.

Employees are presented with the following choices:

Employee Acknowledgement:

Employees complete and sign this form during the enrollment process. Signed copies are retained by Onamac Industries as records of consent
